Confirming user deletion logs administrator out instead of performing deletion

Squash-TM v1.12.1 - I am trying to delete a user; this can be any of the processes defined in the manual - after clicking the "Remove" button, the confirmation dialog opens - clicking the "Confirm" button logs me out instead of deleting the user - when I log back in, the user is still there. The user is NOT able to logon to the system - even thought the password is reset and entered correctly, it always reports "Login failed : login or password incorrect".
